What To Do & Bring On First Visit

Do not have anything to eat or drink the morning before surgery

You must be accompanied by your parent or guardian if you are under the age of 18

You will need to have a responsible driver take you home after surgery
- Bring a completed patient registration form
- Bring a referral slip from your dentist

Bring a list of current medications including any homeopathic you are taking

Bring your dental insurance card and information if you have insurance coverage
- Bring your driver’s license
X-rays
If your dentist have your current X-rays, you would need to have him/her send them to our office via email or regular post mail. If there is not enough time, you can bring them with you to our office. If additional films are necessary, they can be taken at our office.
